Brief Description:
Provide screening and assessment services to clients.
This position will appropriately and professionally complete a thorough assessment on each applicant referred to our agency. This assessment will address the individual’s current strengths and needs. Assessment information is maintained in confidence as per HIPAA requirements. Knowledge-based education provided to the consumer and informal support regarding long-term care options in a timely manner.
Care plan development which addresses an individualized package of home and community-based services relevant to the consumer’s strengths and needs. The care plan utilizes available community resources and does not supplant the role of informal support. This position documents consumer strengths and needs, goals, and interventions as needed. Documentation is expected to be clear, concise, and objective; records are completed in a timely and accurate manner; records are maintained in confidence as per HIPAA requirements.
Key Assessor Responsibilities:
- Provide education for the consumer and informal supports on long-term care options.
- Develop holistic, individualized, care plans that meet consumer/caregiver needs to enable individuals to be de-institutionalized or prevent institutionalization.
- Work collaboratively with providers, informal supports, providers, community agencies, and medical professionals.
- Maintain records and complete associated documentation.
- Complete appropriate referrals on behalf of the consumer.
- Identify health and safety risks, assist in providing direction/resources to prevent harm to the individual.
- Administration of PAS/ID 3622 process for PASRR applying PASRR regulations. Reviews PAS/ID 3622 and Level of Care documentation and issues appropriate Level of Care
Minimum Requirements:
- Degree in Nursing AND RN license OR bachelor’s degree in social work or equivalent AND LSW
- One-year prior experience in home health care, medical social work, or geriatrics
- Knowledge of senior programs, Medicaid rules and regulations, assessments, and geriatrics
- Knowledge of case management and crisis intervention
- Ability to work with a variety of people with different backgrounds, expectations, and education levels.
