Provide screening and assessment services to clients.

Care Managers are involved in completing thorough assessments of consumers who are referred for services, determining consumer needs and developing care plans, and providing knowledge-based educational resources and informal support regarding long-term care options. Care Managers are expected to be appropriate and professional. All assessment information must be maintained in confidence as per HIPAA requirements.

While developing a care plan, the Care Manager creates an individualized package of home and community-based services relevant to the consumer’s strengths and needs. Services and referrals are to be made in a timely manner in accordance with our Quality Assurance and Program Standards. Documentation provided by Care Managers is expected to be clear, concise, and objective. Records must be completed in a timely and accurate manner and maintained in confidence as per HIPAA requirements. Care plans are updated based on scheduled home visits and communication with the consumer and the contracted providers.

Key Care Manager RN/LSW Responsibilities:

  • Provide ongoing education for the consumer and informal supports regarding long-term care options.
  • Develop holistic, individualized care plans that meet consumer/caregiver needs enabling individuals to be de-institutionalized or prevent institutionalization.
  • Monitor, adjust and modify care plans to meet consumer needs on an ongoing basis.
  • Order services and make collateral referrals as per care plan interventions.
  • Work collaboratively with providers, informal supports, providers, community agencies and medical professionals.
  • Maintain records and complete associated documentation.

Minimum Requirements:

  • Degree/Diploma/Certification in Nursing ANDRN license or Bachelors degree in Social Work or equivalent AND LSW
  • One-year prior experience in home health care, medical social work, or geriatrics
  • Knowledge of senior programs, Medicaid rules and regulations, assessments, geriatrics
  • Knowledge of case management and crisis intervention
  • Ability to work with a variety of people with different backgrounds, expectations, and education levels
