Disciple Maker Index

The Disciple Maker Index is a 95-question survey tool that supports Church ministry with objective data, helping Her leaders recognize their potential and envision future possibilities through a deep analysis of attitude and belief, participation, relationship/satisfaction, and demographics.

Components of the Disciple Maker Index

Attitudes and Beliefs

Parishioners are asked about their attitudes toward their parish and their understanding of core Catholic Church teachings.

Relationship and Satisfaction

Parishioners are asked to share feedback on their overall parish experience, the sense of hospitality, and their pastor's leadership.

Participation

Parishioners are asked to provide feedback about the frequency with which they participate in different Catholic practices like Mass, reconciliation, prayer, etc.

Demographics

Parishioners are asked about their attitudes toward their parish and their understanding of core Catholic Church teachings.

All parishioners in the Archdiocese of Cincinnati are invited to take part in the Disciple Maker Index survey. Your participation will help provide your pastor and parish leaders with a better understanding of where to focus efforts and energies to help you grow in your faith and achieve the parish’s mission.

To take the DMI survey for your parish, search for your parish or find your parish name from the list below. If you have any questions about the survey, please contact your pastor or parish office.

In 2026-2027, the responses to the Disciple Maker Index will be included in the listening process for Synod 2027. Archbishop Casey, the synod Preparatory Commission  and Secretariat will review and contemplate the input received from the Disciple Maker Index as they discern the primary themes or questions that will be considered at the Synod General Assembly in fall 2027.  Learn more about Synod 2027.
