Catholic Charities Southwest Ohio is currently accepting applications for a Full Time Preferred Communities Case Manager in our Refugee Resettlement Services program.

As the Preferred Communities Case Manager—Refugee Resettlement Services, you will provide trauma-informed Intensive Case Management and family-centered interventions to ORR-eligible individuals experiencing any of the following vulnerabilities:  single parents/guardians, clients with physical disabilities or medical conditions requiring ICM, clients with mental health needs, elderly and homebound clients, youth who have spent many years in refugee camps, survivors of domestic violence or gender-based violence, secondary migrants new to the area, and clients who need social adjustment supports.

This position will work out of our Cincinnati MidPointe office location (7162 Reading Rd, Cincinnati, OH 45237) normal working schedule is Monday-Friday, 8:30am-4:30-pm, and reports to the Case Management Supervisor, Refugee Resettlement Services.

Job Responsibilities:  

• Provide case management services in accordance to the Preferred Communities Program Plan, including:
• Complete initial assessment and submit to USCCB through MRIS online system within 7 days of client enrollment
• Complete interim assessment every 90 days and submit to USCCB through MRIS online system by the 104^th day
• Complete closing assessment and submit to USCCB through MRIS online system within 14 days of case closure date
• Record external referrals in agency client database system
• Develop self-sufficiency service plans in conjunction with client
• Meet with clients on caseload at least once every 30 days (but more often as appropriate for clients with exceptional needs) for at least 6-12 months or until self-sufficiency is reached
• Maintain caseload of 15-30 clients.
• Connect clients to community resources to help address needs and benefit their daily living (i.e. food pantries, thrift stores, faith communities)
• Ensure case files, both paper and electronic, are kept up to date with the required documentation as set forth in the Cooperative Agreement and program guidelines.
• Complete case notes for all client interactions and ensure information is entered into the program database in a timely manner per documentation policy procedures.
• Maintain communication and cultural understanding with clients, between clients and partners, the Agency, and the community.
• Work collaboratively with student interns and volunteers.
• Assist with preparation of program reports.
• Collaborate with Refugee Resettlement Services staff as needed to ensure that clients are receiving the services they need to achieve their goals.
• Other duties as assigned

Required Qualifications and Skills:

• Bachelor degree in social work or related field
• Demonstrated ability to conduct client assessments, develop service plans, implement interventions, and evaluate service effectiveness
• Knowledge of local health care system and community resources
• Demonstrated ability to work with a variety of cultures, partners and groups; excellent communication skills, organizational and facilitation abilities; Flexibility and adaptability to work with varying client needs and clients from other cultures
• Must be accurate, detail oriented and organized
• Ability to work well in team environment
• English required, foreign language skills a plus.
• Proficient in computer-based client database systems and Microsoft Office Suite.
• Valid driver’s license

 Preferred Qualifications and Skills:

• At least two years’ experience providing case management services
• State of Ohio LSW
• Experience working in refugee resettlement or with refugee/immigrant populations
• Proficiency in a foreign language native to refugee populations, such as Swahili, Kinyarwanda, Arabic, French, Tigrinya

 Work Environment:

Typical office environment, with minimal exposure to excessive noise or adverse environmental issues

 Physical Demands:

Must be able to meet the following physical requirements, with or without reasonable accommodation:

• Use hands to manipulate, handle, feel, and control items or equipment;
• Talk to other employees/clients and hear them;
• See and be able to read, write, and interpret written documents;

This position coordinates client job placement, employer recruitment and engagement and assists with tailored job training classes to promote client self-sufficiency.

• Works with targeted businesses and industry to develop potential worksites and employment opportunities for clients. Negotiates work site training agreements or special accommodations.
• Conducts direct job placement and interviews in coordination with clients and employers.
• Monitors and tracks success following job placement with both refugees and employers in accordance with federal guidelines and serves as a resource to assist with post-placement transition.
• Complete intake and assessments on clients assigned to case load within required time frames.
• Develop family centered self-sufficiency plans for clients on assigned case load. Assist clients in achieving the targets identified in the plan by working with them to eliminate barriers.
• Develop individual employment plans for adult members of assigned case load that take into account client’s work history, language skills, residence, family status, job market and more.
• Ensures case files, both paper and electronic, are kept organized and up to date with the required documentation, documenting the entire client employment process; contributes to time sensitive reports and statistics in accordance with agency guidelines and funding requirements on all services provided.
• Completes case notes for all client interactions and ensures information is entered into the program database and the State database systems in a timely manner per documentation policy procedures.
• Serves as primary contact person for employers and maintains database of current and potential business partners.
• Contributes information and support for the creation of job training classes and community partnerships that matches local employer needs and promotes job placement for clients with limited English proficiency.
• Updates the RSS state webtool weekly for RSS eligible clients.
• Organizes, orientates, and utilizes volunteers, mentors and outside presenters as appropriate and necessary.
• Assists as needed with barrier removal and client preparation for employment to enhance their job placement potential and employment retention, including transportation training to employment placement.
• Assists employers in explaining employment benefits to clients and assists clients in notifying Job and Family Service of changes in employment.
• Collaborates with Refugee Resettlement Services staff as needed to ensure that clients are receiving the services they need to achieve their goals.
• Attends all appropriate meetings.
• Performs other duties as assigned.

Required Qualifications and Skills:

• Bachelor’s degree in social services, marketing, community relations, business or related field; or equivalent work experience.
• Must have excellent public speaking and record keeping skills.
• Ability to work with multicultural caseload and ethnically diverse staff and serve as strong advocate for refugees.
• Ability to work well in team environment and provide direct client services as needed.
• Strong communication skills, flexibility and patience is a must.
• Prior experience with job development, job counseling/training and/or social services provisions.
• Ability to develop and nurture employer contacts.
• English required, foreign language skills a plus
• Proficient skill in utilization of computer-based client tracking and Microsoft Office
• Valid Driver’s License.
