Brief Description:

Assist Refugees and Asylees to become self-sufficient through employment-based case management services and develop employer relationships who will hire refugee clients. Also provides assistance to secondary migrants as capacity allows.

KEY RESPONSIBILITY AREA MAIN DUTIES (Not all inclusive)

        Refugee Employment Assessment and Counseling. (35%)

1. In consultation with the resettlement team, identifies employable refugees that are eligible for employment services considering such factors as prior education and employment history, carries and annual case load of between 125-150, identifies and provides support services to barriers for successful employment
2. Supports employable refugees, identifies employment-related issues for each refugee, manage internal and external referrals to address barriers to employment
3. Assesses the employability of each refugee and evaluates their employment interest
4. Facilitate Job Development, Job Readiness/Placement Assistance, Post-Placement Assistance, and Job Upgrades/Professional Recertification in preparation for job placement and ongoing support for the development of job skills and employment
5. Meets individually and/or in small groups for resume development, assistance with on-line job applications and web-based job searches, assistance with applications for community training programs and accompanying refugees to job interviews

   Employment Attainment and Retention support. (35%)

6. Provides transportation of refugee to job interview and pre-interview job coaching
7. Assists refugee and potential employer, as appropriate, during the interview process
8. Provides debriefing and further coaching upon completion of the interview process
9. Provides appropriate assistance during orientation and initial employment period
10. Ensures that the refugee receives follow-up in early employment period to assess and address work-related issues as appropriate
11. Provides support services to employers during initial employment period
12. Informs case managers, when necessary, regarding barriers to successful employment and plans are made to address such barriers

     Employer Development. (10%)

1. Identifies potential employers
2. Builds working relationships with appropriate employers
3. Maintains a database of relevant information about employers with any openness to hiring refugees
   
   Recordkeeping/Case Recording. (10%)
4. Maintains and organizes case logs and other relevant paperwork pertaining to client records which are obtained by case managers daily
5. Maintains and organizes data for reporting in a timely manner and completes reports by deadlines
6. Tracks client employment placement data and submits relevant information
7. Maintains quality assurance standards
8. Ensures that work is well-prepared, easy-to-read and completed according to program procedures.

   Teamwork and Professional Growth. (10%)

1. Actively participates in team process during meetings and informal processes or projects provided
2. Brings issues for discussion to team or appropriate team member for discussion
3. Participates in ongoing evaluation process of the program provided
4. Seeks supervision toward professional development
5. Assures own professional development by attending workshops, trainings and completion of literature review
6. Demonstrates attitude and approach of “team player”

 Minimum Requirements:

• Bachelor’s degree in Social Work, Business or related field
• Basic computer skills and familiarity with MS Word and Excel
• Ability to understand various accents and levels of English spoken by limited-language speakers
• Ability to communicate with limited English-speaking clients, including having the expertise to speak to clients through interpreters and translators
• Patience in working with limited language speakers and those learning a new culture
• Sound interviewing and employment development skills
• Knowledge of community resources as they relate to employment
• Ability to work with diverse populations
• Ability to network with other persons, businesses, or agencies to meet client needs/goals
• Willingness to work flexible hours
• Valid Ohio driver’s license and access to private, insured vehicle during working hours

Provide screening and assessment services to clients.

This position will appropriately and professionally complete a thorough assessment on each applicant referred to our agency. This assessment will address the individual’s current strengths and needs. Assessment information is maintained in confidence as per HIPAA requirements. Knowledge-based education provided to the consumer and informal support regarding long-term care options in a timely manner.

Care plan development which addresses an individualized package of home and community-based services relevant to the consumer’s strengths and needs. The care plan utilizes available community resources and does not supplant the role of informal support. This position documents consumer strengths and needs, goals, and interventions as needed. Documentation is expected to be clear, concise, and objective; records are completed in a timely and accurate manner; records are maintained in confidence as per HIPAA requirements.

Key Assessor Responsibilities:

• Provide education for the consumer and informal supports on long-term care options.
• Develop holistic, individualized, care plans that meet consumer/caregiver needs to enable individuals to be de-institutionalized or prevent institutionalization.
• Work collaboratively with providers, informal supports, providers, community agencies, and medical professionals.
• Maintain records and complete associated documentation.
• Complete appropriate referrals on behalf of the consumer.
• Identify health and safety risks, assist in providing direction/resources to prevent harm to the individual.
• Administration of PAS/ID 3622 process for PASRR applying PASRR regulations. Reviews PAS/ID 3622 and Level of Care documentation and issues appropriate Level of Care

Minimum Requirements:

• Degree in Nursing AND RN license OR bachelor’s degree in social work or equivalent AND LSW
• One-year prior experience in home health care, medical social work, or geriatrics
• Knowledge of senior programs, Medicaid rules and regulations, assessments, and geriatrics
• Knowledge of case management and crisis intervention
• Ability to work with a variety of people with different backgrounds, expectations, and education levels.

Care Managers are involved in completing thorough assessments of consumers who are referred for services, determining consumer needs and developing care plans, and providing knowledge-based educational resources and informal support regarding long-term care options. Care Managers are expected to be appropriate and professional. All assessment information must be maintained in confidence as per HIPAA requirements.

While developing a care plan, the Care Manager creates an individualized package of home and community-based services relevant to the consumer’s strengths and needs. Services and referrals are to be made in a timely manner in accordance with our Quality Assurance and Program Standards. Documentation provided by Care Managers is expected to be clear, concise, and objective. Records must be completed in a timely and accurate manner and maintained in confidence as per HIPAA requirements. Care plans are updated based on scheduled home visits and communication with the consumer and the contracted providers.

Key Care Manager RN/LSW Responsibilities:

• Provide ongoing education for the consumer and informal supports regarding long-term care options.
• Develop holistic, individualized care plans that meet consumer/caregiver needs enabling individuals to be de-institutionalized or prevent institutionalization.
• Monitor, adjust and modify care plans to meet consumer needs on an ongoing basis.
• Order services and make collateral referrals as per care plan interventions.
• Work collaboratively with providers, informal supports, providers, community agencies and medical professionals.
• Maintain records and complete associated documentation.

Minimum Requirements:

• Degree/Diploma/Certification in Nursing ANDRN license or Bachelors degree in Social Work or equivalent AND LSW
• One-year prior experience in home health care, medical social work, or geriatrics
• Knowledge of senior programs, Medicaid rules and regulations, assessments, geriatrics
• Knowledge of case management and crisis intervention
• Ability to work with a variety of people with different backgrounds, expectations, and education levels
