Assists older adults living with mental illness, to be more active and less isolated through participation in social and recreational activities. This position reports directly to the Guardianship Chums Program Director, and interacts with volunteers, staff, clients, community, and others as necessary. The successful candidate must possess empathy, sensitivity, patience, good listening and communication skills. OhioKAN is an innovative statewide Kinship and Adoption Navigator Program that supports formal and informal kin caregivers and adoptive parents by providing information and referral services and assistance obtaining support services. Building on current and previous Ohio kinship navigator programs, OhioKAN is distinguished by its planned statewide implementation, broad population of families served, and provision for rigorous evaluation. Contingent on evaluation results, OhioKAN may establish a new evidence-based practice model that may be considered for federal payments under the Family First Prevention Services Act of 2018.

OhioKAN builds the necessary community and systems capacity to support families and works directly with kinship and adoptive families to build confidence and connections. Anticipated outcomes to be measured include greater family resilience, improved family functioning, increased stability and permanency for children, and improved child well-being.

As a member of the Hispanic Outreach team, the Hispanic Outreach Front-Door Navigator & Administrative Specialist will provide bilingual (English/Spanish) support and information to families and individuals calling or walking into the agency, with the purpose of connecting callers/clients to the existing family of programs within Catholic Social Services of the Miami Valley, as well as other community resources to assist in eliminating barriers and improving daily functioning. This position requires engagement in case support activities such as brief screenings and assessments and tracking of programmatic activities. As a member of the Administrative Team at the Center for Families, this position supports the building Front-Door Coordinator in reception and monitoring of building activities and takes an active role in building and safety activities.

This position provides Care management for clients in the COMCARE program, as well as supportive services to the Clinical Manager and quality assurance unit. This position is responsible for developing person-centered care plans which address an individualized package of home and community-based services relevant to the consumer’s strengths and needs. Services and referrals are made in a timely manner in accordance with Quality Assurance and Program Standards.
